Prior to entering the 7th grade, all students must provide an updated Florida Certificate of Immunization. Seventh grade students lacking proper immunization documentation will not be allowed to attend school. Please review the form prior to turning it in to the front office because forms with errors will not meet state requirements.
Here are some common errors…
- Immunization dates that are prior to the child’s date of birth.
- Missing dates for required vaccinations.
- Certificate fails to indicate, “Immunizations are complete for 7th grade”.
- Tetanus Booster not specified as Tdap or Td (FYI: State requires Tdap)
- Form not signed by physician.
- Turning in copies of the form. Please note,only originals are accepted.

Children in need of immunizations can visit the Broward County Health Department’s mobile health van at our school on the following dates and times….
February 9th, 12th, 16th and 19th from 9 am -1 pm. A parent must accompany the student and you need to bring your immunization records with you.
