Please note that the Florida Department of Health, Bureau of Immunization has announced a new immunization requirement effective for the 2009/2010 school year for students entering the 7th grade.
Students entering, repeating or transferring into 7th grade will need to provide the school with a blue immunization form documenting that they have received the Tdap vaccine and are cleared for 7th grade entry. If your child received the Td vaccine instead of the Tdap vaccine, you must now provide the school with a blue immunization form stating that your child has a temporary medical exemption with an expiration date no greater than 5 years from the date the Td vaccine was received.
Avoid the VERY LONG LINES at orientation and ensure that your child is allowed to start the 7th grade by turning in your childís original blue immunization form today. During the summer, the office is open Monday through Thursday from 7:30 am-4:30 pm.
